SHBG during weight loss: what actually moves?

SHBG is the protein that binds testosterone and oestradiol in your blood. Its full name is sex hormone binding globulin. Anything stuck to this protein cannot be used by your tissues.

So your SHBG decides how much of your total testosterone is actually working. There is a second reason to measure it during a weight-loss programme. The main driver of a low SHBG is a persistently high insulin level.

That travels with insulin resistance and fat mass. As that improves while you lose weight, your SHBG usually rises. So this value moves with the exact thing your programme is changing.

SHBG (Sex Hormone Binding Globulin): what this test measures

SHBG is a protein that binds sex hormones in your blood and carries them around. Its full name is sex hormone binding globulin. Your liver produces it.

SHBG is not a hormone itself. It is a transport protein.

SHBG binds testosterone and oestradiol, but not equally firmly. The bond with testosterone is strong, the one with oestradiol clearly looser. Anything attached to this protein cannot be used by your tissues.

Your total testosterone adds bound and unbound hormone together. Your SHBG decides how that total splits. That is why free testosterone is calculated from the pair.

Results are reported in nmol/l. The normal range depends on your sex and on your age group.

Look closely at the second row. SHBG rises with age in men, so the ceiling climbs to 76.7 nmol/l. A man of 60 with a value of 70 sits inside his own band.

Judge him against the younger band and the same result looks high. The reverse also holds: 19 looks normal while his floor is 20.6 nmol/l. One flat band per sex simply does not work here.

Every laboratory sets its own band, depending on the analyser it runs. We work with a network of independent, certified laboratories across the Netherlands. They do not all run the same equipment, so the bands vary. The values printed on your own report therefore outrank the table above.

For you there is one more layer. A table tells you where a population sits, not where you started. During a weight-loss programme your own baseline is the reference point you need.

There is one reason SHBG belongs on a metabolic panel: it moves with your insulin balance. The dominant cause of a low SHBG is a persistently high insulin level. That high insulin travels with insulin resistance and with excess fat tissue.

Your liver then produces less SHBG. As your insulin sensitivity improves while you lose weight, your SHBG usually climbs back.

That makes this value unusual on a panel. Most blood values describe an organ or a store. SHBG describes the process your programme is trying to change.

So have your fasting insulin and your HbA1c measured in the same draw. When all three move the same way, you read one story instead of three loose figures.

The second reason is your testosterone. Without SHBG a testosterone result cannot be read properly, and this runs in both directions. A low SHBG pulls your total testosterone down while your free testosterone stays normal.

A high SHBG does the opposite: the total holds up, but less free hormone remains. Say you carry a lot of excess weight and test only a total value. That easily produces a misleading figure.

The same applies to a woman using the pill.

There is a third point, and it needs care. Across large population studies, a low SHBG is linked to more type 2 diabetes. That holds in men and in women.

It is a link between groups of people, nothing more. A blood test cannot predict disease and does not make a diagnosis. So do not read a low value as a verdict on your future.

Treat it as a reason to measure your glucose control alongside it.

In practice this means the direction tells you more than the figure. Take a baseline before you start, then repeat at around three and around six months. Has your SHBG gone from 22 to 34 nmol/l while your weight fell?

That is a meaningful movement. Both figures can sit comfortably inside the same reference band. Without that baseline you would never have seen the change.

  • Water is always fine. Drink as usual beforehand, it makes the draw easier.
  • Take your usual medication, unless your own doctor told you otherwise.

With SHBG what matters for you is a series of measurements, not a single draw. The cadence that suits a supervised weight-loss programme is straightforward. Take a baseline before you start. Measure again at around three months, and once more at around six months.

That first measurement matters most, and it is the one most often skipped. Without a baseline you cannot tell later whether 34 nmol/l is a rise or a fall. That direction is exactly the information you are after.

Request your fasting glucose in the same draw, plus your total testosterone. Free testosterone is calculated from that pair, so the two belong together on the form. A total testosterone value without SHBG is hard to interpret. That is especially true with a lot of excess weight.

Test in the morning and fasted, so your glucose and insulin are usable. SHBG itself is not especially sensitive to the time of day. Do not measure shortly after an infection, an operation or a spell of illness. Such a data point contaminates your series without anything in your metabolism having changed.

Use the same laboratory each time where you can. Different analysers work with different bands. You would then be comparing figures on two different scales. If the laboratory does change along the way, note that next to your result.

Low Levels

A low SHBG usually causes no symptoms you can feel. What you notice belongs to the cause behind it. The list of things that lower SHBG is short.

Excess weight and insulin resistance come first, followed by type 2 diabetes. An underactive thyroid and non-alcoholic fatty liver lower it as well. So do glucocorticoids, androgens and anabolic steroids.

In a weight-loss programme you are usually in that first group. Think of tiredness after meals, weight that will not shift, low energy and a growing waistline. These complaints are non-specific and can have dozens of causes.

On their own they prove nothing. In men a low SHBG has one practical consequence as well. Your total testosterone then comes out lower than your free testosterone justifies.

That can look like a deficit when it is not one. So always have a low value assessed by your doctor. Bring your testosterone values and your glucose control along.

High Levels

If your SHBG is high, you usually feel nothing from it directly. It is a signal rather than a complaint. The known causes are an overactive thyroid and liver cirrhosis.

Oral oestrogens count too, such as the combined pill and oral hormone therapy. Severely restricted energy intake raises it as well, as in anorexia. Some anti-epileptic drugs do the same, and SHBG simply climbs with age.

With transdermal oestrogen you do not see this effect. During a strict weight-loss programme a large energy deficit can lift your value as well. If you also notice hair loss, feeling cold or lasting fatigue, discuss that with your doctor.

The same goes for periods that stop. With a high value, one question matters most. How much free testosterone is actually left?

Low SHBG may indicate metabolic syndrome, obesity, or insulin resistance. Focus on weight loss, exercise, and metabolic health optimization.

If High

High SHBG may cause symptoms despite normal testosterone. Consider hyperthyroidism evaluation and consultation with an endocrinologist.

Female

Low SHBG may indicate PCOS, insulin resistance, or metabolic syndrome. Consider weight management, low-glycemic diet, and PCOS evaluation.

The lever under your SHBG is your insulin sensitivity. Anything that improves it tends to push your SHBG the right way. Weight loss is the strongest factor here, and losing visceral fat around your organs counts most. That is exactly what a supervised programme aims at, so you are already working on it.

Resistance training and daily movement help on top of that, independently of the scales. Muscle tissue takes up glucose without much insulin being involved. Sleep counts too, because sleeping short measurably worsens your insulin sensitivity.

Alcohol loads your liver, and your liver is the organ that produces SHBG. Cutting back on alcohol is therefore not a separate tip but the same lever.

On a sharply reduced intake, watch your protein and your fibre. That helps you hold on to muscle while your weight falls. Muscle is what clears glucose. Take your triglycerides in the same draw, because they respond to insulin resistance as well.

Two things are better left alone. Do not take supplements to steer your SHBG. No product does that in a targeted and safe way.

And never change anything about a prescribed medicine yourself on the basis of a blood value. If a result does not match your expectation, discuss it with your doctor. Look at the whole series, not one point.

What happens to my SHBG when I lose weight?

In most people it rises. The dominant cause of a low SHBG is a persistently high insulin level. As your insulin sensitivity improves while your weight falls, your liver makes more SHBG again.

How fast that happens and how much it shifts varies a lot between people. That is why your own series tells you more than an average from a study.

Why does the direction of my SHBG say more than the number?

Because the reference band is wide and describes an entire population. If you go from 22 to 34 nmol/l, something has clearly changed. A single isolated result does not show that change.

So compare your value with the normal range for your age group. Then set it against your own starting value. The second comparison is usually the more useful one.

When should I measure SHBG during a GLP-1 programme?

Take a baseline before you start, then repeat at around three and around six months. Test fasted and in the morning. That keeps the glucose and insulin from the same tube usable.

Skip a measurement point if you have just been ill. Such a point contaminates your series without anything in your metabolism having changed.

What should I measure alongside SHBG?

Total testosterone always belongs with it, because free testosterone is calculated from that pair. Add fasting glucose, fasting insulin and HbA1c to that. Together those four show whether your insulin resistance is moving with your weight. An SHBG value without that context stays an isolated figure you cannot get much direction from.

My SHBG stayed inside the reference range, so did nothing happen?

Not necessarily. The band is wide, so you can move a long way inside it without leaving it. A step from 40 to 60 nmol/l stays entirely within the band for a woman.

It is still a real change. That is precisely why a baseline is worth so much during a programme.

What does a high SHBG mean during strict weight loss?

A large energy deficit can lift your SHBG, as can an overactive thyroid or oral oestrogens. What you mainly want to know is how much free testosterone is left. So request your total testosterone alongside it. If the value stays high, or symptoms appear as well, discuss the result with your doctor.

Does a low SHBG predict that I will get diabetes?

No. Large population studies do link a low SHBG to more type 2 diabetes. That link runs between groups of people, not to you personally.

A blood test cannot predict disease and does not make a diagnosis. What it does do is give you a reason to measure your glucose control alongside it.

Why does my total testosterone not match how I feel?

Your SHBG may be the reason. A low SHBG pulls your total value down while your free testosterone stays normal. A high SHBG does exactly the reverse.

Without SHBG you therefore do not know which part of your total testosterone is available. Leave the interpretation to your doctor, with all three values in front of them.
