An elevated lipase usually does not mean pancreatitis. The enzyme leaks from your pancreas when the gland is irritated. It also rises when kidney function drops, and with certain medicines.
Sometimes there is no traceable reason at all. How far above the limit your value sits is what matters. That says far more than the word elevated on its own.
In acute pancreatitis, lipase normally sits at three times the upper limit or higher. A value just above the line more often belongs to something harmless.
One thing almost no page mentions. People on GLP-1 medication see this value rise regularly without anything being wrong.
What does lipase actually do?
Lipase is a digestive enzyme your pancreas makes to break down fat. The pancreatic duct carries it into your duodenum. That is where it belongs, rather than in your blood.
A small amount of lipase in the blood is normal. When more appears, enzyme is leaking out of the gland tissue. How much leaks is the real signal.
Amylase does something comparable for carbohydrates and comes from the same gland, plus from your salivary glands. Both enzymes normally circulate in small amounts in your blood.
That last part makes amylase the less reliable of the two. An inflamed salivary gland lifts your amylase while your pancreas has nothing to do with it. So when the pancreas is the question, laboratories prefer lipase over amylase.
When is lipase too high?
Normal values at most Dutch laboratories sit below 60 to 70 U/l. The Dutch Society for Clinical Chemistry gives an upper limit of around 70 U/l. It does vary by lab and by method.
Always compare your result against the reference range printed on your own report. How far above that range you sit is what determines the meaning.

That three-times threshold is agreed internationally. It comes from the Atlanta classification of acute pancreatitis (Banks et al., 2013). That classification also asks for abdominal pain matching the picture, or for findings on a scan. So one raised result is not a diagnosis.
How fast does lipase rise, and how long does it stay high?
The time course partly determines what your result is worth. In acute inflammation, lipase in the blood rises quickly.
The peak usually falls around 24 hours after symptoms begin. By 48 hours that rise is almost always visible. The value then stays elevated for another five to seven days.
At its peak, lipase is often five to ten times higher than in healthy people. So when the blood was drawn matters too. A result taken a week later may already have fallen back.
Why does lipase rise on GLP-1 medication?
Because these drugs act on the pancreas directly. The LEADER trial followed 9,340 participants for nearly four years. In people taking liraglutide, lipase rose by an average of 28% and amylase by 7%. The rise happened in the first six months and then stayed stable (Steinberg et al., 2017).
That is a large number in a large trial.
The most important result, though, is not the rise. Confirmed acute pancreatitis occurred in 0.4% on liraglutide against 0.5% on placebo. Fewer, not more. And those raised values did not predict the inflammations: the positive predictive value stayed under 1%.
An elevated lipase in someone on a GLP-1 drug is therefore an expected effect of the medicine. It is not a warning.

Does an elevated lipase mean you should stop?
That is not a question a blood value can answer by itself. The Dutch Farmacotherapeutisch Kompas names persistent severe abdominal pain, often radiating to the back. That is the complaint that makes a doctor think of the pancreas. The result without those symptoms means something different from the result with them.
Symptoms weigh more here than the number does.
Take two people with exactly the same result of 95 U/l. The first has been on a GLP-1 drug for four months and feels fine. The second has had severe abdominal pain pulling through to the back since yesterday.
The number is identical. For the first, the value fits the drug and following it is enough. For the second, it needs assessing that same day.

Without those symptoms, a mildly raised value is something to follow. It is not something to panic about.
Your doctor will weigh what your kidneys are doing. With reduced kidney function, lipase clears from the blood more slowly. Your creatinine belongs in the same assessment.
What if your lipase is normal, or low?
Not everyone with a pancreas problem has a raised lipase. Normal values do not fully rule a problem out. In chronic pancreatitis, lipase activity is usually not raised. That condition damages the gland, which over time produces less enzyme.
A low value fits a gland making too little digestive enzyme. Fatty stools, weight loss and bloating belong with that picture. Discuss that pattern with your GP, even when your result sits inside the reference range.
Which other values belong with it?
The ones sharing the same anatomy. Your bile ducts and liver sit right against the pancreas. ALT and gamma-GT tell you whether the problem is there instead. A sharply raised ALT alongside abdominal pain points towards the bile ducts rather than towards your medicine.
Alongside those, triglycerides belong in the set. Very high triglycerides are themselves a cause of pancreatitis, and they usually fall during weight loss.
There is one more reason to take gallstones seriously during a weight-loss course. A Swedish study followed 6,640 people. Those on a very-low-calorie diet developed symptomatic gallstones more than three times as often. The figures were 152 against 44 per 10,000 person-years (Johansson et al., 2014).
The Dutch Maag Lever Darm Stichting names gallstones and alcohol as the two most common causes. That is for acute pancreatitis in the Netherlands. Medication sits far behind them.

What is worth measuring before you start?
A baseline, because everything later gets compared against it. A lipase of 90 U/l says little without that comparison. Starting at 40 is a different situation from starting at 80. Anyone who first draws the value once symptoms are there misses exactly that comparison.
That is why amylase and lipase sit in all of our panels.

What I would look at myself
The combination, not the isolated number. Take a lipase at twice the upper limit in someone with no abdominal pain. That person has just started a GLP-1 drug. It is a completely different thing from the same value alongside severe pain radiating to the back.
What strikes me about this value is how often it frightens people unnecessarily.
The LEADER figures are more reassuring on that point than almost anything else written about it. Have your baseline measured before you start. A later result then becomes a comparison instead of a puzzle.

Can stress raise lipase?
Stress is not on the usual list of causes. Alcohol, gallstones, certain medicines and reduced kidney function do lift the value. So does GLP-1 medication, without anything being wrong.
