How fast does Mounjaro work? A timeline from week 1 to week 72

How fast Mounjaro works depends on what you measure. Appetite drops for most people within a few days of the first injection. The scale follows in weeks. The bulk of the weight loss only arrives over the course of more than a year.

In the SURMOUNT-1 trial average weight loss rose to 20.9% after 72 weeks at the highest dose (Jastreboff et al., 2022).

Those 72 weeks are rarely mentioned. Most Dutch pages quote the final figure without the time attached to it, and that skews what people expect.

Below is the timeline per phase, and what can be measured in each.

How quickly do you notice Mounjaro?

The first effect is reduced appetite, usually within three to seven days of the first injection. Tirzepatide slows stomach emptying, so you feel full sooner. Weight loss only becomes visible on the scale for most people after two to four weeks. This timeline is an average picture and says nothing about your own pace.

When does the scale move?

For most people after two to four weeks, and slowly. The 2.5 mg starting dose exists to let your body adjust, not to lose weight. Only once the dose steps up does the decline usually speed up. In SURMOUNT-1 the curve was still falling well into the second year.

That is the hardest phase for many people. Week six feels like little result for a lot of side effect.

Say you are two kilos down after a month on 2.5 mg. That is not a sign the drug is failing, because you are still in the build-up.

When do your blood values move?

Faster than the scale, in the case of fasting insulin. It can fall within two to four weeks, because your intake drops immediately. HbA1c responds slowly by contrast: it reflects your average blood sugar over roughly eight to twelve weeks, so before week eight it says little.

That difference in pace is often overlooked. An HbA1c measured in week three largely reflects the period before you started.

Fasting insulin is often more informative in those first weeks. In SURPASS-2 tirzepatide produced a larger reduction in HbA1c than semaglutide 1 mg over 40 weeks (Frias et al., 2021).

The storage values come last of all. Ferritin and vitamin B12 need months before a lower intake becomes visible.

Why is it faster for one person than another?

Dose is the biggest factor, and it steps up differently for everyone. Beyond that, starting weight, how much someone moves, how much protein they eat and how strong the side effects are all play a part. Someone who barely eats for two weeks because of nausea loses weight faster, but also loses more muscle mass.

That last point is why speed is not always good news.

In the SEMALEAN study fat mass fell by 14% at seven months and 18% at twelve months, while lean mass first dropped by 3 kilos and then held steady. Handgrip strength rose over that same period (Alissou et al., 2026).

The Dutch Voedingscentrum points out that weight loss goes together with loss of muscle mass and that adequate protein plays a role, citing 1.0 to 1.2 grams per kilogram of body weight for healthy older adults.

What can you record in the meantime?

The practical answer is the starting point, and then a second measurement once the values have had time to move. For HbA1c that means at least eight to twelve weeks. No schedule fits everyone, and your GP can think along about what makes sense in your situation.

Week six is, to my mind, where most people give up. I think that is a shame, because by then more has usually happened in the blood than on the scale.
